Dr. Cook is born in Sublette, Kansas to Curtis Carl and Carmen Madonna Cook.

Dr. Cook receives an Associate of Arts from Hutchinson Community College.
1978
Dr. Cook marries Paula Rose Petryszyn.

Dr. Cook receives a Bachelor of Arts from the University of Kansas.
1983
Dr. Cook receives a Doctor of Medicine from the University of Kansas.
1983
Dr. Cook becomes a resident in diagnostic radiology at the University of Kansas Medical Center.
1985
Dr. Cook receives the Resident Teaching Award from the University of Kansas Department of Radiology.
1987
Dr. Cook goes into private practice in Corpus Christi, Texas and becomes a staff radiologist at Spohn Memorial Medical Center.
1989
Dr. Cook becomes chairman of the management commission of the South Texas Hospital System.

Dr. Cook becomes chairman of the management committee at Radiology Associates in Corpus Christi, Texas.

Dr. Cook becomes medical director of Del Mar College Ultrasound Technological School and chief of Corpus Christi Medical Center.
