Dr. Middleton in born to Anthony and Dolores Middleton.
1963
Dr. Middleton receives a Bachelor of Science from the University of Utah.
1966
Dr. Middleton receives a Doctor of Medicine from Cornell University and begins a medical internship at the University of Utah Hospital within University of Utah Health.
1968
Dr. Middleton serves in the U.S. Air Force, earning the rank of Captain.
1969
Dr. Middleton receives a medical license in the state of Utah.
1970
Dr. Middleton marries Carol Samuelson and begins a residency in urology at Massachusetts General Hospital.
1974
Dr. Middleton begins practicing in urology at Middleton Urological Associates.
1977
Dr. Middleton serves as chairman of the Division of Urology at the Salt Lake Regional Medical Center.
1977
Dr. Middleton becomes an associate clinical professor of surgery in the School of Medicine at the University of Utah.
1985
Dr. Middleton serves as chairman of the Utah Medical Self-Insured Association.
1993
Dr. Middleton receives the Distinguished Service Award from the Utah Medical Association.
1996
Dr. Middleton serves as chairman of the medical advisory board of UroQuest Medical Corp.

Dr. Middleton receives the Distinguished Service Award from the American Association of Clinical Urologists, Inc.

Dr. Middleton is appointed medical director of Utah-Idaho Lithotripsy.

Dr. Middleton receives the Outstanding Service Award from the American Urologic Association.

Dr. Middleton serves as a consultant and a physician for Intellectual Reserve, Inc. of the Church of Jesus Christ of Latter-day Saints.
